A shopping mall in central China's Hubei Province is responsible for an escalator accident that killed a woman on Sunday morning, local authorities announced Monday night.

A woman died on Sunday morning after falling through a gap that suddenly opened at the top of an operating escalator in a shopping mall in Hubei's Jinzhou City, with horrifying security camera footage of the incident going viral online.

According to a statement by local government, shopping mall staff noticed that a panel at the top of the escalator was loose five minutes before the accident, but no one repaired it.

The accident happened at 9:57 a.m. Sunday when 31-year-old Xiang Liujuan stepped on the end of the loose panel and was dragged into a gap that suddenly opened. She pushed her son out of the way to be rescued by a mall staff member.

One staff member tried to grab Xiang but failed.

The cause of the loose panel is under investigation. Endi
